Course Registration Forms can be found on our Registrar site.

  • Be sure to select forms designated for “MPP” students
  • The “Request to Enroll in Fuqua course” form should only be used after Academic Programs has communicated enrollment procedures for Fuqua. Please contact if you have questions.
  • Note: These forms are electronic and will route themselves to the appropriate approver automatically. Once the appropriate approvers have signed the form and the request has been processed, you will be notified by Sanford Registrar/Assistant Registrar.  Please allow several business days for the forms to be signed and processed.
